Replacing a Paycheck
Lost or Stolen Checks
- Immediately notify your Payroll Department and request that a stop payment be put on your check. Your Payroll department will ascertain the document number and submit a Stop Payment Notice to Corporate Payroll Services, who will notify NYC Health + Hospitals’ bank and request issuance of a stop payment.
- Complete a Lost/Stolen and Forged Check Affidavit, have it notarized, and return it to your facility’s Payroll Department. Your Payroll Department will forward the completed affidavit to Corporate Payroll Services for processing.
Once the completed affidavit is received and NYC Health + Hospitals’ bank confirms that the check is outstanding and the stop has been placed, Corporate Payroll Services will replace the check, generally within 2-3 business days.
If the check has already been cashed:
If NYC Health + Hospitals' bank discovers that the check has already been cashed, they will provide a copy of the canceled check, which your Payroll Department will forward to you to verify the signature. If the endorsed signature is not yours, you will be asked to complete a second Lost/Stolen and Forged Check Affidavit, certifying that the signature on the check is not yours, that it was not authorized by you, and that you did not receive any part of the proceeds of the check.
The notarized Lost/Stolen and Forged Check Affidavit will be submitted to the bank of issuance for investigation. If the investigation proves that the check signature is a forgery, NYC Health + Hospitals will be reimbursed, which in turn will issue you a replacement check. This process generally takes at least two months.
Damaged or Stale-Dated Check
NYC Health + Hospitals' payroll checks are valid for 180 days from the check date. NYC Health + Hospitals will not replace a check that is valid. If your paycheck becomes damaged, NYC Health + Hospitals will replace it. You must return the damaged or stale dated check to your facility's Payroll Department in order for a replacement check to be issued. A replacement check can usually be issued within three business days.
